Immunization Information
Parents of all 6th grade students please do not forget to bring proof of Tdap and MCV4 (meningitis) immunization shots by the first day of school. You may request an exemption from the nurse as necessary.
All new students must have proof that immunizations are current and up to date or have a signed exemption form at time of registration, or first day of attendance.
IF YOU WOULD LIKE TO DONATE ITEMS TO THE NURSE OFFICE THE FOLLOWING WOULD BE GREATLY APPRECIATED.
1. Clorox or Lysol wipes.
2. Feminine hygiene products.
3. Cereal bars or graham crackers.
4. Juice boxes.
5. Water bottles.
PLEASE NOTE THAT DUE TO THE NUMBER OF PEANUT AND TREE NUT ALLERGIES ALL CLASSROOMS ARE DESIGNATED AS PEANUT FREE AREAS. Exceptions are student lunches from home.
